Shipping Instructions:
Fill in the form below and print it out to send
your media to Mason-Dixon Concierge, LLC. The top part helps us determine what we will charge for all the work you select
to be done.
Send no money or credit card information with your order. We will
contact you by telephone with our final charges and get your payment instructions at that time.
After
processing, your original photographs plus any DVD's and tapes will be returned by UPS with $100.00 insurance to the address
below unless you advise us differently. If we determine that the total charges are going to exceed $ 250.00 we will not run
the job without first obtaining your permission. Normal turnaround is two weeks.
Please organize
your pictures sequentially and number them. We Will follow your numbers when transfering the photos and slides. ship any instructions,
your media and this form to:
